This brief is part of a series of PCMH case studies. While ourother case studies have focused on individual clinic-level innovation, thisbrief provides a cross-state comparison of PCMH programs. The Arkansas HealthCare Payment Improvement Initiative (AHCPII) has positioned the state as aleader in payment innovation by restructuring the system to align incentivesthat reward value and quality outcomes. The state’s activities are in alignmentwith goals established by the Centers for Medicare and Medicaid Services (CMS)to tie the majority of care to value-based payments by 2018. Patient Centered Medical Homes (PCMHs) are a primary strategy of AHCPII, with design andimplementation of the state’s PCMH efforts being led by Arkansas Medicaid withsupport from Arkansas Blue Cross and Blue Shield, QualChoice, Humana,Centene/Ambetter, Medicare, Walmart, the State and Public School EmployeeBenefits Plans, and others. Arkansas’s multi-payer program has demonstratedsuccess and established Arkansas as a national PCMH leader. Other states haveimplemented PCMH programs with varying results.
